New Dog Tag Application/Replacement Tag Form

All dogs must be registered in the Municipality of Arran-Elderslie. A dog tag must be attached to the dog's collar at all times. If you get a new dog please come into the Municipal Office to register your dog and to purchase a tag. It is your responsibility to notify the office if there is any change to your pet's information, you are moving out of the Municipality, or you no longer have a dog. You can view dog tag fees in our Fees By-Law.

After submission of this form you will receive a call or email from the municipality to come in to pick up your tag and invoice. This form may be used for a new tag request or a replacement tag request.
