New Dog Tag Application/Replacement Tag Form

All dogs must be registered in the Municipality of Arran-Elderslie.  A dog tag must be attached to the dog's collar at all times.  If you get a new dog please come into the Municipal Office to register your dog and to purchase a tag. It is your responsibility to notify the office if there is any change to your pet's information, you are moving out of the Municipality, or you no longer have a dog. When your dog is registered with us, you will receive an invoice and a permanent dog tag in the mail.  This form may be used for a new tag request or a replacement tag request.
